  • I prefer to receive my UL Reports and Procedures via the following method (select one, please). I understand this will replace existing mailing methods established for all contacts receiving UL Report or Procedure for the above Files at my company.

  • If you have not yet registered for your free MyHome @UL account, Register Now and request access to your company's records via the "Access your company's Subscriber Information" link on the main page of MyHome @UL.

    For assistance completing your MyHome registration > click on the Help button > Quicktips > under Gatekeeper, click Establish a Gatekeeper. You may also contact Customer Service for assistance.

    PLEASE NOTE: You will be responsible to ensure the UL Field Representative has access to the Follow-Up Service (FUS) Procedure documentation (either electronically or hard copy) during each FUS visit per your Follow-Up Services Agreement.
