Instructions for completing UL Form CS-ASD-FAS
Fire alarm certificate request
Note: Issue dates can't be more then 30 days in the past from the time they are received at UL. All certificates may be issued from one to five years in length.
Page 1
- Protected property -- This section should contain the name of your alarm service subscriber and the complete address of property in which the system is installed.
- Alarm service company -- This section should contain all the information about the alarm service company responsible issuing the certificate.
- Period of issuance -- Issue date is the calendar date that the UL certificate is considered to be in force. Expiration date is the calendar date on which the systems UL's certification is considered expired. These dates may range in length from one to five years, but may not exceed five years. This section also contains reference to the type of certificate you are attempting to issue. If applicable, provide the serial number of the certificate that you are renewing or replacing, to speed up the process.

- Comments and clarifications -- This section is used when there is additional information about the system that cannot be found elsewhere on the certificate. Text will appear on the certificate exactly as written in the space provided.
- Area covered -- This section should contain the building(s), floor(s) and other area(s) covered by the certificate when active.
- Alarm system description system type -- This section contains specific information about the system and the code/standard by which it is identified. Indicate your choice by circling the type, NFPA code/standard and providing us the edition year. Also required in this section is providing both the authority having jurisdiction (AHJ) as well as the responding fire department.

- Automatic fire detection and alarm service -- Circle the type of system you wish to certify (total, selected or partial). Note that a system type of "partial" will require the deviations from the NFPA to be listed on the certificate; please provide those on page 5 of the form, under system deviations. Selected area coverage should be explained under the comments section located on page 2.
- In the space below the type of coverage, please provide the total number of detectors (smoke, heat, duct, or other) as well as their types in the allotted spaces. It is important that the total numbers add up.
- Sprinkler system waterflow alarm and supervisory service -- Show type of coverage. If all sprinkler risers and water shutoff valves at protected property are not supervised, describe all particulars under system deviations on page 5 of the worksheet. List all quantities of devices connected in the system.
- Also indicate point of connection to other suppression systems in this field.

- Manual fire alarm and guard's tour supervisory service -- List the quantity of all devices installed. If the proper number of fire alarm boxes required by the referenced NFPA standard is not installed, list the particulars in the system deviations on page 5.
- Alarm notification and annunciation devices -- Show quantity of devices installed. Indicate the type of visual and or audible/visual signals, making sure to include the specific device type. If quantity and location of devices does not comply with the NFPA referenced standard, list the particulars in the system deviations on page 5.
- Emergency voice alarm service -- Show quantity of devices installed. If quantity and location of devices does not comply with the referenced NFPA standard, list the particulars in the system deviations on page 5.

- System deviations from referenced NFPA standards -- The specific NFPA standard that was referenced on page 2 which provides requirements for the installation, location, coverage, periodic testing, maintenance and response for all devices comprising the alarm system and the required service. If any aspect of the system does not comply with the NFPA standard, the details must be shown in this section. In addition you should retain in your records a copy of the authorization from the authority having jurisdiction (AHJ) for the deviation listed.
- Control and transmitter units -- Indicate the manufacturer's name and model number of the system control unit(s). If an off-premises signal-transmitting device is employed which is separate from the system control unit, show the manufacturer's name and model number of this device. Do not show manufacturer's name and model number of equipment such as initiating or notification devices.
- Alarm transmission method -- Indicate the transmission for off-premises communication from the protected property to the monitoring facility by selecting the appropriate method.

- Remote monitoring -- Circle the appropriate box for the type of monitoring being used: UL Listed central station, fire department dispatch center, proprietary supervising station, AHJ approved location, or none. Provide all necessary information about the monitoring entity. For a UL Listed central station, provide the file number and service center number if known (the monitoring company will have this information). All of the above listed forms of monitoring should include a primary retransmission method for the signal. Check the appropriate retransmission method from the list provided.
Note: For locations approved by AHJ, there must be a document available for review, indicating that the AHJ has approved the alternate monitoring location.
