- Title of Standard
- Scope of Standard
- Reason for the New Standard
- Where will most of the requirements come from? Such as another existing UL Standard, other Organizations Standards, International Standards, or new requirements that need to be developed specifically for this product.
- If using existing Standards, please list.
- If an organization is requesting a new Standard, provide Name, address, telephone number, email address and contact person's name for the organization.
- Are there existing industry groups related to this product? If so, please name as many as known.
- Are there any government agencies related to this product? If so, please name as many as known.
