Certificate request forms
ULC listed alarm, signal receiving and software companies will use the links below for the forms to request a new ULC certificate. The forms are available in Microsoft Word format.

All requests for Shared Service certificates shall have a signature (or email address) from both the alarm company, as well as the alarm monitoring company, on the Certificate Request Form, prior to the submitting the request to ULC for processing.
All Full Service certificate requests require only the signature of the authorized person from the alarm company requesting the certificate.
Expiring ULC certificates will be automatically renewed and emailed at least one month prior to the expiration date. Modifications or cancellations of existing certificates should be handled via email and do not require completion of a request form. Please include the certificate number, occupant name and address in the email.
